No. You don't ever have to take a drivers test. You can take it any time after you reach the required age and/or meet the driver's ed. requirements. Good luck.
I am not aware of any law on the books in Iowa that requires anyone to take a drivers test on their birthday. So the answer is no, you could go the rest of your life and never take a drivers test if you don't want to.
And if you do want to take one, it won't be on your birthday unless you happen to have scheduled one for that date.
