i know you need the mv-265 if you are not 16 and i know that a parent signature is required, but whata about the chart on the bottom of that sheet…do i need to fill that out as well?
because some of that information i don’t know

If you’re talking about the grid that’s labeled something like "instructor/date/length/school" or something like that, you only need that filled out when a driving teacher is keeping track of the amount of practice you’ve had. Just have a guardian sign in and you’ll be fine; that’s what I did. Have it signed before going to the test.
